A total of 222 customers who paid by check within 75 days prior to May 7, 2009.

A burglary occurred at the business and a safe with checks from customers along with cash was stolen. Letters were sent to those affected. The safe was recovered and it appears the information was undisturbed.

A laptop computer was stolen from an employee’s home that contained personal information. Free credit monitoring services will be offered for all of those affected.

Who’s affected
Details

More than 30,000 Lexis Nexis customers. Those affected have been offered one year of credit monitoring free from ConsumerInfo.com.

This letter from LexisNexis was sent to potential victims whose information may have been compromised.

In the notification letter to those affected Lexis Nexis says thieves used “fraudulently - opened mail boxes at commercial mail - receiving businesses” and personal information via Lexis Nexis to obtain credit cards. Approximately 300 individuals are known victims so far. The investigation is continuing to disclose details of the case.
Those affected should review their credit reports and monitor personal accounts and report any fraud immediately to law enforcement.

UC Berkeley University Health Services

Who’s affected
Details

Approximately 160,000 individuals [38 Wisconsin residents] who either attended a Berkeley facility, clients and/or spouses or parents of students were affected.

Those affected have been notified and asked to place a fraud alert on their credit report. A copy of the letter can be found below:

Evidence suggests hackers involved had access to the databases from October 2008 through April 6, 2009 when it was discovered by UHS staff. The databases involved were removed from service. An investigation is ongoing.
