Details

Currently 17 Wisconsin residents have been identified as those affected by a compromise of user information that can lead to personal information including SSN, bank account information and account balances.

Quixtar, Inc. identified a compromise to user account information and an attempt to transfer funds. Letters were sent to those affected explaining what occurred and what actions should be taken to protect themselves.

Who’s affected
Details

Approximately 2.2 million patients and guarantors, including some Wisconsin residents.

A metal box of back up tapes was stolen from a car driven by an independent storage company employee. The Salt Lake City PD, FBI and Secret Service are investigating the theft.
Those affected are receiving letters offering one free year of credit monitoring services.
